Recent fetal ultrasound and Mother's Date of birth ( Attached Adhaar Card ) is required and any prior pregnancies information with trisomy 21. Also submit a properly completed Maternal Serum Screen request form. Done at 14.1-22 week. USG report is Mandatory.
