Overview

Celiac disease is associated with a variety of autoantibodies, including endomysial, tissue transglutaminase (tTG), and deamidated gliadin antibodies. Although the IgA isotype of these antibodies usually predominates in celiac disease, individuals may also produce IgG isotypes, particularly if the individual is IgA deficient. The most sensitive and specific serologic tests are tTG and deamidated gliadin antibodies. The disease is HLA-associated (>95% of patients have DQ2 & DQ8 HLA marker) and manifests at any age with a peak onset in early childhood, even in neonates. The determination of IgG antibodies to tTG is the only available specific serology for those 2% to 5% of patients with IgA deficiency.

What is TTG-IgA Test ?

Components of TTG-IgA Test

Tissue Transglutaminase Immunoglobulin A (TTG-IgA): Tissue Transglutaminase Immunoglobulin A (TTG-IgA) antibodies are generated by the immune system in reaction to gluten consumption in celiac disease. These antibodies cause inflammation and damage to the lining of the small intestine by targeting the enzyme tissue transglutaminase. The presence of TTG-IgA antibodies is a key marker used to diagnose celiac disease.

What is the Purpose of the TTG-IgA Test ?

The main purpose of celiac disease antibody testing is to assist in the diagnosis and follow-up of celiac disease in individuals exhibiting symptoms such as anemia and abdominal pain. Approximately 4% to 12% of people have or will acquire celiac disease, hence screening for the condition in asymptomatic family members may sometimes be done using coeliac testing. This testing may also be advised for those who have other autoimmune diseases.

Interpreting the TTG-IgA Test Results

All positive and indeterminate celiac disease tests are typically followed by an intestinal biopsy to make a definitive diagnosis of celiac disease. About 2%-3% of patients may test negative for serologic studies. If celiac disease is still suspected, a biopsy is generally done.

If you haven't eaten gluten for a few weeks or months before the test, your results for celiac disease may be negative. If your doctor still suspects the presence of celiac disease, they might put you through a gluten challenge, which involves reintroducing gluten into your diet for a few weeks or months to observe whether your symptoms reappear. At that point, a biopsy to look for damage to the intestinal villi may be performed or celiac disease testing may be repeated.

When is a TTG-IgA Test Recommended ?

How is the TTG-IgA Test Done ?

The Blood Sample for TTG-IgA Test is taken in following steps:
  • The area where the needle will be inserted to take your blood is cleaned with an antiseptic alcohol wipe. The hollow on the inside side of the elbow or the tip of the finger are the most common sites for this.
  • A tourniquet or a band is tied around your upper arm to facilitate the access to the vein in your arm.
  • A needle is used to puncture your vein. The blood sample is collected into a test tube attached to the needle. The needle may cause a small pinch or sting when it is inserted.
  • Once the vial or test tube is filled, the blood sample collection for the test is finished and the needle is taken out.
  • Finger pressure is applied on the puncture site to stop the bleeding and the band aid is applied at the site.

Preparation for TTG-IgA Test

  • Unless otherwise directed by your doctor, there is no need for you to make any special preparations before the TTG-IgA Test. 
  • Other blood tests can be carried out in addition to this test. If your doctor advises you to have multiple tests at once, they may ask you to fast for a specific period of time prior to a blood test.
  • Drink adequate water before sample collection.
  • Follow the preparatory instructions as guided by your doctor.
