Overview

Prothrombin time is used to identify a deficiency of one or more of the clotting factors of the extrinsic coagulation system (I, II, V, VII, X) due to hereditary deficiency or acquired conditions such as liver disease, vitamin K deficiency, or a specific factor inhibitor. Monitoring patients on oral anticoagulant therapy to maintain a patient in a safe therapeutic range.The prothrombin time is also reported as the INR (International Normalized Ratio), based on the ISI (International Sensitivity Index) Draw 6 mL of whole blood in 2 blue-top (Sodium Citrate) tube and gently mix by inverting. Transport the sample to the lab within 4 hours. If this isn’t possible, prepare PPP within 1 hour: First, centrifuge the sample at 3600 rpm for 15 minutes, then transfer the supernatant to a clean plastic tube. Centrifuge this supernatant again at 3600 rpm for another 15 minutes, and transfer the final supernatant (PPP) into a LABELED, clean plastic screw-capped vial. FREEZE IMMEDIATELY and ship FROZEN. DO NOT THAW.

What is Prothrombin Time Test ?

What is the Purpose of Prothrombin Time Test ?

Preparation for Prothrombin Time Test

  • Unless otherwise directed by your doctor, there is no need for you to make any special preparations before the prothrombin time test. 
  • Other blood tests can be carried out in addition to a PT. If your doctor advises you to have multiple tests at once, they may ask you to fast for a specific period of time prior to a blood test.
  • Drink adequate water before sample collection.
  • Avoid vigorous physical activity before the sample collection.
  • Follow the preparatory instructions as guided by your doctor.

Components of Prothrombin Time Test

  • Patient Plasma: This blood sample is being tested to find out how long it takes for a clot to form, which indicates how well the person's blood can clot.
  • Mean Normal Prothrombin Time (MNPT): The standard clotting time of the general population, against which the patient's clotting time is assessed.
  • Prothrombin Ratio (PR): The patient's Prothrombin Time to MNPT ratio, which suggests possible clotting anomalies.
  • International Normalized Ratio (INR): A standardized PR version that adjusts test variability in order to ensure consistent blood-thinning therapy monitoring among various labs.

Interpreting the Prothrombin Time Test Results

The normal ranges for PT/INR results are:
  • 11 to 13.5 seconds
  • INR of 0.8 to 1.1

How is the Prothrombin Time Test Done ?

The Blood Sample for Prothrombin Time Test is taken in following steps:
  • The area where the needle will be inserted to take your blood is cleaned with an antiseptic alcohol wipe. The hollow on the inside side of the elbow or the tip of the finger are the most common sites for this.
  • A tourniquet or a band is tied around your upper arm to facilitate the access to the vein in your arm.
  • A needle is used to puncture your vein. The blood sample is collected into a test tube attached to the needle. The needle may cause a small pinch or sting when it is inserted.
  • Once the vial or test tube is filled, the blood sample collection for the test is finished and the needle is taken out.
  • Finger pressure is applied on the puncture site to stop the bleeding and the band aid is applied at the site.
