Overview

Serum glutamate pyruvate transaminase (SGPT), also known as alanine aminotransferase (ALT), is an enzyme primarily found in the liver. Elevated SGPT levels may indicate liver cell damage due to conditions such as hepatitis, fatty liver, alcohol use, or liver injury. This test is commonly used to assess liver health and monitor ongoing liver conditions.

What is the S.G.P.T (ALT) Test ?

Alanine Aminotransferase (ALT) is present primarily in Liver cells. In Viral Hepatitis and other forms of Liver disease associated with Hepatic Necrosis, Serum ALT is elevated even before the clinical signs and symptoms of the disease appear. Although serum levels of both aspartate aminotransferase (AST) and ALT become elevated whenever disease processes affect liver cell integrity, ALT is a more liver-specific enzyme. Serum elevations of ALT are rarely observed in conditions other than parenchymal liver disease. Moreover, the elevation of ALT activity persists longer than does AST activity.

How is the S.G.P.T (ALT) Test done ?

The Blood Sample for ALT test is taken in following steps:
  • The area where the needle will be inserted to take your blood is cleaned with an antiseptic alcohol wipe. The hollow on the inside side of the elbow or the tip of the finger are the most common sites for this.
  • A tourniquet or a band is tied around your upper arm to facilitate the access to the vein in your arm.
  • A needle is used to puncture your vein. The blood sample is collected into a test tube attached to the needle. The needle may cause a small pinch or sting when it is inserted.
  • Once the vial or test tube is filled, the blood sample collection for the test is finished and the needle is taken out.
  • Finger pressure is applied on the puncture site to stop the bleeding and the band aid is applied at the site.

What is the Purpose of the S.G.P.T (ALT) Test ?

The ALT test is advised for following analysis:
  • Screening: If you are suffering from obesity, diabetes, habit of heavy alcohol consumption, or have a family history of liver disease, your doctor may advise you on an ALT screening. ALT levels can be tested during regular medical examinations along with complete LFT.
  • Diagnosis: This test may also be advised after symptoms like jaundice, fatigue, swelling, unexplained weight loss, itching, nausea, vomiting, or other symptoms associated with liver problems have appeared.
  • Monitoring: After the diagnosis of a liver disease, ALT test is often advised to monitor the course of the disease and the efficacy of therapy over an extended period, ensuring appropriate management when required.
  • Assessing the Effect of Medical Conditions and Treatments: ALT test helps to evaluate how different medical conditions and treatments, like chemotherapy or other medication, affect ALT levels, ensuring the best possible liver care and overall treatment efficacy.

When is a S.G.P.T (ALT) Test Recommended ?

Components of S.G.P.T (ALT) Test

  • S.G.P.T. / ALT: This test helps to measure the levels of alanine aminotransferase. Alanine aminotransferase is an enzyme that is majorly present in the liver. Since ALT is secreted into the bloodstream when liver cells are damaged, elevated levels are indicative of inflammation or liver damage. Liver function evaluation and diagnosis greatly depends on the measurement of ALT.

Preparation for S.G.P.T (ALT) Test

  • Unless otherwise directed by your doctor, there is no need for you to make any special preparations before the ALT test. 
  • Other blood tests can be carried out in addition to an ALT test. If your doctor advises you to have multiple tests at once, they may ask you to fast for a specific period of time prior to a blood test.
  • Drink adequate water before sample collection.
  • Avoid vigorous physical activity before the sample collection.

Interpreting the S.G.P.T (ALT) Test Results

If your results are abnormal, you will likely be advised to go for further testing as an ALT test cannot firmly diagnose a particular condition. Your doctor can help you interpret your results. Normal values for the particular test are typically displayed along with your numbers. Following are the normal values of ALT levels:
  • Alanine transaminase (ALT): 0 to 45 IU/L
High blood levels of ALT can be due to damage or injury to liver cells. Such levels may indicate the following conditions:
  • Alcohol-induced liver injury
  • Fatty liver disease
  • Hepatitis 
  • Cirrhosis 
  • Taking medications that are toxic to your liver.
  • Liver tumor
  • Liver ischemia 
  • Hemochromatosis 
  • Mononucleosis 
  • Certain genetic conditions can affect your liver
