The Intact PTH test measures levels of parathyroid hormone (PTH), which is secreted by the parathyroid glands to regulate calcium and phosphorus balance. Elevated PTH levels are common in primary hyperparathyroidism, though some patients may present with normal levels despite high calcium. The test helps diagnose calcium-related disorders and assess parathyroid function, with low serum phosphorus often accompanying elevated PTH.

What is the Intact Parathyroid Hormone (PTH) Test ?

Parathyroid Hormone (PTH) is produced and secreted by the Parathyroid Glands, which are located along the posterior aspect of the Thyroid Gland. The Hormone is synthesized as a 115-amino acid precursor (Pre-Pro-PTH), cleaved to pro-PTH, and then to the 84-amino acid molecule, PTH (Numbering, by universal convention, starting at the amino terminus). The precursor forms generally remain within the Parathyroid cells. About 90% of the patients with primary hyperparathyroidism have elevated Parathyroid Hormone (PTH) levels. The remaining patients have normal (Inappropriate for the elevated Calcium level) PTH levels. About 40% of the patients with primary Hyperparathyroidism have serum phosphorus levels below 2.5 mg/dL, and about 80% have serum phosphorus below 3.0 mg/dL.

What is the Purpose of the Intact Parathyroid Hormone (PTH) Test ?

This test is advised if there are following observations from your Blood tests:
  • Too much or too little calcium in the bloodstream
  • Low levels of phosphorus in the blood
  • Severe osteoporosis

Interpreting the Intact Parathyroid Hormone (PTH) Test Results

PTH level is higher than normal (Hyperparathyroidism):

Primary Hyperparathyroidism is high PTH levels with high blood calcium levels due to following reasons:

  • A benign tumor of parathyroid glands
  • Hyperplasia of your parathyroid glands
  • Multiple endocrine neoplasia type 1 (familial hypocalciuric hypercalcemia)
  • Parathyroid Cancer

Secondary Hyperparathyroidism is high PTH levels with low blood calcium levels.

Possible causes include:

  • Low levels of vitamin D (vitamin D deficiency), phosphate, and/or magnesium
  • Poor calcium intake
  • Digestive disorders, such as malabsorption syndromes
  • Kidney failure

IPTH levels are lower than normal Blood Calcium level is also low):

  • Damage to your parathyroid glands from surgery on your neck or radiation therapy for cancer
  • Certain autoimmune disorders affecting parathyroid tissue
  • Certain genetic disorders, such as DiGeorge syndrome

How is the Intact Parathyroid Hormone (PTH) Test Done ?

The Blood Sample for Intact Parathyroid Hormone (PTH) Test is taken in following steps:
  • The area where the needle will be inserted to take your blood is cleaned with an antiseptic alcohol wipe. The hollow on the inside side of the elbow or the tip of the finger are the most common sites for this.
  • A tourniquet or a band is tied around your upper arm to facilitate the access to the vein in your arm.
  • A needle is used to puncture your vein. The blood sample is collected into a test tube attached to the needle. The needle may cause a small pinch or sting when it is inserted.
  • Once the vial or test tube is filled, the blood sample collection for the test is finished and the needle is taken out.
  • Finger pressure is applied on the puncture site to stop the bleeding and the band aid is applied at the site.
