Overview

Alkaline phosphatase consists of several types, primarily liver-bone-kidney, and is found in various tissues, including bones and liver. Elevated levels are seen in cholestasis, especially obstructive jaundice, and in bone diseases like Paget disease, hyperparathyroidism, rickets, osteomalacia, fractures, and certain cancers.

What is the Alkaline Phosphatase Test ?

Alkaline Phosphatase in serum consists of 4 structural Genotypes: Liver-Bone-Kidney, intestinal, and placental types, and the variant from the Germ Cells. It is found in Osteoblasts, Hepatocytes, Leukocytes, the Kidneys, Spleen, Placenta, Prostate, and the small intestine. The Liver-Bone-Kidney type is particularly important. A rise in the Alkaline Phosphatase occurs with all forms of Cholestasis, particularly with obstructive jaundice. It is also elevated in diseases of the skeletal system, such as Paget disease, Hyperparathyroidism, Rickets, and Osteomalacia, as well as with fractures and malignant tumors.

What is the Purpose of the Alkaline Phosphatase Test ?

This test is advised for following purposes:
  • Screening: It involves detecting health problems even before the appearance of symptoms associated with a range of potential health problems.
  • Diagnosis: It helps in detecting the disorders after the appearance of signs of possible liver problems, such as nausea and vomiting, jaundice, fatigue, abdominal pain, or unexplained weight loss. It is also often advised if you have symptoms of a possible bone disorder, such as bone pain, susceptibility to bone fractures, or abnormal bone shape. 
  • Monitoring: It may provide information about the severity of the disease and how a condition changes over time with treatment.

When is an Alkaline Phosphatase Test Recommended ?

The following are a few signs that could indicate the need for an Alkaline Phosphatase Test: 
  • Nausea 
  • Vomiting
  • Poor appetite

  • Swelling and/or pain in your abdomen

  • Drink adequate water before sample collection.

Components of Alkaline Phosphatase Test

  • Alkaline Phosphatase (ALP): ALP is a protein that functions as an enzyme to accelerate specific chemical reactions in your body. Although ALP is present in every bodily tissue, it is more abundant in the liver, bile ducts, and bones. Every bodily organ produces a distinct kind of ALP.

Interpreting the Alkaline Phosphatase Test Results

The normal range for Alkaline Phosphatase (ALP) levels can vary slightly depending on the age, sex, and overall health of the individual. The normal ranges of ALP are:
  • Adults: 44 to 147 IU/L (international units per liter)
  • Children and Adolescents: 120 to 320 IU/L (due to bone growth and development)
  • Pregnant Women: May have higher levels due to placental ALP production.
High alkaline phosphatase levels can be due to:
  • Blockages in your bile ducts
  • Cirrhosis
  • Hepatitis
  • Mononucleosis, which can sometimes cause swelling in the liver
Moderately high levels of ALP can be due to:
  • Hodgkin lymphoma
  • Heart failure
  • Certain infections
Low levels of ALP are less common but may indicate:

How is the Alkaline Phosphatase Test Done ?

The Blood Sample for Alkaline Phosphatase Test is taken in following steps:
  • The area where the needle will be inserted to take your blood is cleaned with an antiseptic alcohol wipe. The hollow on the inside side of the elbow or the tip of the finger are the most common sites for this.
  • A tourniquet or a band is tied around your upper arm to facilitate the access to the vein in your arm.
  • A needle is used to puncture your vein. The blood sample is collected into a test tube attached to the needle. The needle may cause a small pinch or sting when it is inserted.
  • Once the vial or test tube is filled, the blood sample collection for the test is finished and the needle is taken out.
  • Finger pressure is applied on the puncture site to stop the bleeding and the band aid is applied at the site.
