Overview

The quadruple marker test is a second-trimester prenatal screening that measures four substances in a pregnant woman’s blood: alpha-fetoprotein (AFP), human chorionic gonadotropin (HCG), estriol, and inhibin-A. This test assesses the risk of chromosomal abnormalities like Down syndrome (trisomy 21), trisomy 18, and neural tube or abdominal wall defects in the developing baby.

What is the Quadruple Marker Test ?

The Quad Screen, also known as the Quadruple Marker Test, the second trimester screen, or simply the quad test, is a prenatal test that measures the levels of four substances in a Pregnant woman's blood: Alpha-Fetoprotein (AFP), a protein made by the developing baby; Human Chorionic Gonadotropin (hCG), a hormone made by the Placenta; estriol, a hormone produced by the Placenta and the baby's Liver. The Quad Screen evaluates the likelihood of carrying a baby with any of the following conditions: Down syndrome (Trisomy 21), trisomy 18, abdominal wall defects, or neural tube defects.

What is the Purpose of the Quadruple Marker Test ?

This Test is advised for following analysis:
  • Risk Assessment for Genetic Disorders: It helps assess the risk of the baby having conditions like Down syndrome (trisomy 21), trisomy 18, and neural tube defects.
  • Evaluation of Abdominal Wall Defects: The test can identify the likelihood of fetal abdominal wall defects.
  • Second Trimester Screening: It is performed during the second trimester, typically between 14 to 21 weeks of pregnancy.
  • Non-Invasive Screening: The test is a blood test that measures the levels of four substances—alpha-fetoprotein (AFP), human chorionic gonadotropin (hCG), estriol, and inhibin-A—without the need for invasive procedures.
  • Guidance for Further Testing: While it cannot diagnose conditions, it helps determine whether further diagnostic tests (such as amniocentesis) are needed.

When is a Quadruple Marker Test Recommended ?

The Quadruple Marker Test is recommended between 14 to 21 weeks of Pregnancy, with the ideal time being around 16 to 18 weeks. It is typically performed during the second Trimester as part of routine prenatal care.

Preparation for Quadruple Marker Test

  • The test is recommended for women between 14 to 21.6 weeks of pregnancy, with a fully completed maternal TRF that includes a barcode, along with the Ultrasound (USG) report.
  • A detailed medical history is mandatory, and recent Fetal Ultrasound results, along with the mother's date of birth, are required.
  • There is no special preparation required before the Quadruple Marker Test. 
  • Other blood tests can be carried out in addition to a Quadruple Marker Test. If your doctor advises you to have multiple tests at once, they may ask you to fast for a specific period of time prior to a blood test.
  • Follow the preparatory instructions as guided by your doctor.

Components of Quadruple Marker Test

  • Alpha-Fetoprotein (AFP): A Protein produced by the developing Baby’s Liver.
  • Human Chorionic Gonadotropin (hCG): A Hormone produced by the Placenta.
  • Estriol: A Hormone made by both the Placenta and the Baby’s Liver.
  • Inhibin-A: A Hormone produced by the Placenta.

Interpreting the Quadruple Marker Test Results

  • Negative Result (Normal Quadruple Marker Test): A normal (Or negative) result means the Fetus is not at higher risk for Congenital disorders. 
  • Positive Result (Abnormal Quadruple Marker Test): An abnormal (Or positive) result indicates an increased risk for a Congenital disorder, but it does not confirm the presence of a Genetic condition. In this case, further diagnostic testing is usually recommended, such as an additional Ultrasound or amniocentesis, to assess the Baby's health. 

How is the Quadruple Marker Test Done ?

The Blood Sample for Quadruple Marker Test is taken in following steps:
  • The area where the Needle will be inserted to take your blood is cleaned with an Antiseptic Alcohol wipe. The hollow on the inside side of the Elbow or the tip of the Finger are the most common sites for this.
  • A tourniquet or a band is tied around your upper arm to facilitate the access to the Vein in your Arm.
  • A Needle is used to puncture your Vein. The Blood sample is collected into a test tube attached to the Needle. The Needle may cause a small pinch or sting when it is inserted.
  • Once the Vial or test tube is filled, the blood sample collection for the test is finished and the Needle is taken out.
  • Finger pressure is applied on the puncture site to stop the bleeding and the Band aid is applied at the site.
